What does “three-way” mean

Standard order at Cincinnati chili parlors like Skyline and Gold Star: a bed of spaghetti, a ladle of cinnamon-spiced Cincinnati chili, and a mountain of finely shredded mild cheddar. A 'four-way' adds onions or beans, a 'five-way' adds both, and 'six-way' adds garlic. Asking for a three-way anywhere else in the country produces a very different reaction.
